4/7/2024 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Ashiyah appeared to be in a jovial mood evident by her facial expressions as well her overall communication with QMHP-C. QMHP-C inquired about her day. Ashiyah verbalized to QMHP-C that her day was going well. QMHP-C informed Ashiyah that this session was going to focus on ways to control her anger in a controlled manner since she has been struggling with such at which mother reports these behaviors present themselves at least 2 times weekly with 10 minutes in duration. 2. Behavior: During the second portion of the session, Ashiyah displayed inappropriate behaviors such as utilizing profanity when discussing the lack of communication, she has with her mother and those who are in authority at which these behaviors lasted 2-3 minutes in duration until redirected by QMHP-C. 1.Intervention: QMHP-C expressed to the family the role she would play with Ashiyah during each session and what Ashiyah should expect and be prepared for. QMHP-C reiterated to Ashiyah the support she would bring in order to assist Ashiyah in achieving her goals. QMHP-C brainstormed possible ideas for activities to facilitate with Ashiyah that she may be more interested in than the educational worksheets and video presentations. QMHP-C encouraged Ashiyah to continue displaying positive behaviors. QMHP-C supported Ashiyah involvement in selected activities. QMHP-C processed with Ashiyah about what she learned from activity after the review. QMHP-C praised Ashiyah for her cooperation and staying seated during the entire session. 2.Intervention: QMHP-C praised and recognized Ashiyah for her hard work, efforts, participation, and working towards improvement by changing daily habits and implementing a positive and consistent routine. QMHP-C supported Ashiyah with meeting her objectives and coping with stressors without needing additional redirection or prompts. QMHP-C modeled proper positive behavioral expectations for Ashiyah to encourage additional layers of support and understanding of rules and expectations. QMHP-C monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP-C provided individual, group, and family counseling based on specific treatment objectives. QMHP-C encouraged Ashiyah to continue displaying age appropriate behavior, and the benefits of this will be reviewed with parent. 1.Response: Ashiyah and parent reviewed program overview together in a polite manner. Ashiyah emphasized the key points and objectives according to everything she was taught while focusing on her needs and how to be a better person. Ashiyah was respectful of the roles in session. Each family member was able to verbalize how they were feeling and how there day was. Each family member understood the expectations and rules for family session. Ashiyah and mom verbalized that they are doing well with communicating with each other. Ashiyah was receptive and open to the information that was provided by QMHP-C, as she listened attentively to all that was presented to her regarding her behaviors. Ashiyahâs mom was approachable and open-minded to the feedback given by QMHP-C. Ashiyah was able to express her feelings and moods in a positive way without getting upset. Ashiyah followed the instructions and remained focus and active during the therapeutic activity. 2.Response: Ashiyah was cooperative. Ashiyah participated and cooperated well with the social skills activities. Ashiyah was able to answer the thinking questions with assistance from QMHP-C. Ashiyah asked questions to QMHP-C concerning the social skills activities. Ashiyah also verbalized that she displayed positive behaviors in the home. Ashiyah fully participated in role-reversal techniques during the session. Ashiyah was able to demonstrate positive interaction and excellent manners without having an episode. Ashiyah said thank you. Ashiyah displayed high self-esteem and confidence. Ashiyah appropriately answered QMHP-C open-ended questions. Ashiyah was observed practicing breathing and relaxation strategies and mastering the âstop, think, actâ method. Ashiyah was acceptive of QMHP-C support. Ashiyah followed all directions, as she demonstrated ability to adhere to the rules and expectations of individual session.
